The Agency failed to inform patients of the disciplines that will be providing services and the frequency of visits.

The Agency failed to have a Clinical Manager since March 23, 2026. Findings include: 1. During the survey entrance process, it was observed that Agency had no Clinical Manager present.

The Agency failed to ensure staff provide service/treatment as ordered by the Physician and indicated in the POC.

The Agency failed to ensure Pts. were provided with the Administrator’s name and contact information.

The Agency failed to ensure Pts. were informed and/or participated in the disciplines to provide care/service and the frequency of visits provided by each discipline.

The Agency failed to ensure Pts. were made aware of their payment source and/or financial responsibility for care and services at the SOC.

The Agency failed to ensure complete information regarding the State Hotline was provided to Pts.

The Agency failed to ensure the Agency assessed the need for Pneumococcal Pneumonia and/or Influenza vaccines.

The Agency failed to provide information regarding the Clinical Director and their contact information.

The Agency failed to ensure the RN provided accurate written instructions to the Home Health Aide.

The Agency failed to ensure PsOC included all medications, directions for appliance use and patient specific diabetic management.

The Agency failed to coordinate care effectively to ensure the patient's needs were met.

The Agency failed to ensure the RN conducted Home Health Aide supervisory visits every 14 days.

The Agency staff failed to ensure that the comprehensive assessment accurately reflect the patient's integumentary status.

The Agency failed to ensure the information contained in the clinical record are complete and accurate.
