What the severity labels mean
Serious
A condition-level citation. Inspectors found the agency failed a whole Medicare requirement, such as care planning or patient rights. Ask the agency what changed since.
Moderate
A standard-level citation. A specific part of a requirement was not met. Common, but worth asking about if it repeats.
Minor
An element-level citation. A narrower detail was not met, often paperwork or a single step in a process.
62,659 deficiencies are on record nationwide, 3,788 of them serious. 12,460 agencies are in CMS's current list; 1,532 more have past records but are no longer in it. Most agencies have some findings; what matters is how serious they were, whether they repeat, and how recent they are. How to choose an agency →
