The Agency failed to provide annual staff training of the Emergency Preparedness Program since 2022. Findings include: 1. The Emergency Preparedness Program was reviewed on 08/27/25. The program lacked documentation of Agency providing annual staff training on the Emergency Preparedness Program.

The Agency failed to submit reports timely, resulting in a warning.

The Agency failed to screen patients for pneumonia vaccination status on admission according to Agency policy.

The Agency failed to ensure documentation of 100 percent compliance of field staff COVID-19 vaccination.

It was determined that the Agency failed to ensure informed patient consents contained the frequency visits of the disciplines that will furnish care . This was found in 6 of 7 clinical records reviewed.

The Agency failed to ensure the initial assessment visit was done within 48 hours of the referral date or Physician order.

The Agency failed to ensure the Physician was sent a copy of the Discharge Summary.

The Agency failed to coordinate with other emergency officials, or update its Emergency Preparedness plan annually.

The Agency failed to arrange a transfer agreement with a surrounding Agency/facility to ensure continuity of Pt. care in the event of an emergency.

The Agency failed to conduct annual Emergency Preparedness training/testing with staff.

The Agency failed to conduct an annual full-scale community-based/facility based emergency exercise, as well as a second full-scale community/facility-based or table top exercise.

The Agency failed to ensure that the PsOC included frequencies and durations.

The Agency failed to ensure the Home Health Aide provided services included on the Aide care plan.

The Agency failed to complete a transfer summary after the patient's transfer from the Agency.
