The Agency failed to conduct testing of the Emergency Preparedness Plan since 2020. Findings include: 1. The Agency's Emergency Preparedness Plan (EPP) was reviewed on 06/21/23 at 1:00 PM.

The Agency failed to ensure the Staff followed the Physician orders.

The Agency failed to ensure the clinical records were complete.

The Agency failed to establish and maintain a comprehensive Emergency Preparedness Plan ; failed to include individual plans for each patient as part of the comprehensive assessment; failed to develop a communication plan ; and failed to develop a training and testing program.

The Agency failed to ensure the clinical record contained accurate Medication Profiles.
