For 1 of 2 (Pt #7), discharged records reviewed from the agency, the physical therapist (PT) failed to complete visit documentation as required.

For 1 of 7 patients (Pt #4), the Agency failed to ensure the Plan of Care (POC) addressed all of the patient's problems.
