The agency failed to maintain a current list of medications for 5 of 7 patients that received home visits (#10, 9, 3, 7, and 8) ) and 1 of 1 discharged patient record (#3).

The agency failed to document patient specific services provided by home health aides for 5 of 7 patients receiving home health aides (#9, 11, 15, 8, and 17) and failed to document accurate wound measurements and physical therapy missed visit for 1 of 1 patient (#8).

The agency failed to measure the peripherally inserted central catheter (PICC) external catheter length per agency policy in 3 of 3 patient records (# 13, #14, and 7).
