For 1 of 1 (Pt #1) patients records reviewed with oxygen, the Agency failed to ensure staff provided care, as indicated on the physician orders.

For 6 of 7 (Pt #1, Pt #2, Pt #3, Pt #6, Pt #8, Pt #9) patients records reviewed, the Agency failed to ensure Plan of Cares (POCs) were authenticated and dated per policy.

For 3 of 7 (Pt #6, Pt #8, Pt #9) patients records reviewed, the Agency failed to ensure verbal orders were authenticated and dated per policy.

For 1 of 3 (Pt #9) patients records reviewed who were discharged, the Agency failed to ensure a discharge summary was completed and provided to the primary care provider within 5 business days.

For 7 of 7 (Pt #1, Pt #2, Pt #3, Pt #4, Pt #5, Pt #6, Pt #7) patients records reviewed without known or suspected COVID-19, the Agency failed to ensure patients and/or caregivers were screened for COVID-19 signs and symptoms before at the beginning of their home visit.
