The 08/19/19 HHA care plan was incomplete / inaccurate as follows: Bathing The HHA care plan included instructions: Shower if no sponge bath Sponge bath if no shower - The SN failed to specify when to provide a shower vs. sponge bath and how to manage wounds during bathing.
