The agency failed to ensure the patient's plan of care included a description of the patient's risk for emergency department visits and hospital re-admission, plans for discharge and rehabilitation potential for three of seven patient records reviewed (Patient #1, #3, and #6 ).

The agency failed to ensure a supervisory visit was conducted every 60 days to observe the home health aide performing care for one of two non-skilled patient records reviewed (Patient #6).
