Skilled nurse (SN) failed to notify the physician promptly while patient had an elevated BS (blood sugar) during visit for one out of 7 sample patients (4).

The skilled nurse (SN) failed to wash/sanitize his hands after removing the gloves that touched soiled dressing, and prior to providing wound care for one of of seven sample patients (Patient 1).
