The Home Health Agency (HHA) failed to ensure the nurse followed accepted standards of practice for hand hygiene and wound care to prevent infections. 1.On 9/29/26 at approximately 10:30 AM, a home visit was conducted at Pt #2’s residence.

In 1 of 3 (Pt #9) patient discharge charts reviewed, the Home Health Agency failed to provide the patient with resource information of other providers upon discharge.

In 2 of 2 (Pt #8 and Pt #11) records reviewed, the Agency failed to obtain a physician order/POLST order for the DNR status on the POC.. Findings include: 1.Pt #8 SOC: 8/25/2023 Diagnosis: Pyonephrosis.

In 1 of 4 (Pt #10) patients with home health aide (HHA) services ordered, the Agency failed to ensure orders for the HHA visits were followed and failed to notify the physician of a change in the plan of care.

For 1 of 5 (E #1) part time, physical therapy assistant's training records reviewed, the Agency failed to ensure staff were provided appropriate education on COVID-19, potentially affecting all patients and staff at the Home Health Agency with a current census of 63 patients.
