The agency failed to ensure a registered nurse evaluated competency of the home health aide and failed to ensure all of the required areas were evaluated for competency for 1 of 1 home health aide personnel files reviewed (#4).

The agency failed to ensure the patients' individualized plan of care included all of the required elements to address the patients' needs for 7 of 10 (#1, #2, #3, #5, #6, #7, and #9 ) sampled patients.

The agency failed to alert the physician to any changes in the patient's condition or needs that suggest that outcomes are not being achieved and/or that the plan of care should be altered for 1 of 10 ( #6 ) sampled patients.

The agency failed to provide services as ordered by the physician in the plan of care for 2 of 10 (#1 and #9) sampled patients.
