The agency failed to notify the physician of agency discharging patient from home health services for two (2) of twenty (20) CRs reviewed (CR#18, CR#19).

Agency failed to have the total plan of care reviewed by the attending physician at least once every 60 days for five (5) out of twenty (20) clinical records (CR#6, CR#7, CR#10, CR#12, CR#14).

Agency failed to complete the initial assessment within 48 hours of referral, or within 48 hours of the patients return home, or on the physician ordered start of care date for two (2) out of twenty (20) clinical records.
