The Home Health Agency failed to ensure its nursing staff documented supervisory visits for the Home Health Aide (HHA) every 14 days for 1 of 2 applicable patients.

Registered Nurse(RN) 1 failed to use appropriate gloving techniques during wound care to prevent the potential cross transmission of infectious agents in the home setting for one of seven patients observed in the provision of care.

The agency failed to accurately reconcile the medications the patient was currently taking with the agency's Medication Administration Record (MAR)and/or failed to identify medication discrepancies during the drug review for 6 of 7 patients visited in the home.
