The agency failed to implement infection control practices for two of 17 sampled patients (Patient 6 and Patient 12) when: 1. Registered Nurse 4 (RN 4) had artificial fingernails while performing direct patient care to Patient 6.

The agency failed to complete medication reconciliation (each medication individually reviewed as currently being taken at proper doses and following particular physician instructions) for four of 17 sampled patients (Patient 1, Patient 3, Patient 11, and Patient 14) at each skilled home visit.

The agency failed to provide education to one of 17 patients (Patient 6) on infection prevention and environmental safety according to the plan of care.

The agency failed to provide services in the plan of care for one of 17 sampled patients (Patient 9).

The agency failed to notify the physician of patients' missed or canceled visits for two of 17 sampled patients (Patient 4 and Patient 8).

The agency failed to provide home visits as ordered by the physician for one of 17 sampled patients (Patient 10).

The agency failed to complete discharge and transfer summaries for three of 17 sampled patients (Patient 11, Patient 8, and Patient 2).
